This is a project proposal for quality improvement initiative for fall prevention among older adults 65 years and older residing a long-term care setting/nursing home. The participants will be all older adults male and females from 65 and above residing in the facility and/or admitted during the window of the study. Exclusion criteria can like late dementia/bedbound patients. I have attached my previously completed need assessment and instructions. Pls reach out for additional question/concerns.
