Introduction
By the end of this assignment, you will be able to describe patient medical records, physical exams, and observations that need to be assessed to correctly diagnose rashes and lesions. (CO7)

Assignment Guidelines
To help distinguish and diagnose rashes and lesions, thorough health history and physical exam need to be done. What morphological criteria need to be collected and what questions would you include in your focused history? Additionally, sometimes a punch biopsy may need to be completed to further identify/investigate a lesion. Please provide information necessary for this procedure, including:
• Basic steps
• Patient information (side effects, risks, pain, etc.)
• Can a nurse practitioner complete this procedure? Why or why not?
