5-6 Slides with an executive summary in a separate document in APA style.

Create a 5- or 6-slide narrated PowerPoint that presents a comprehensive plan to implement changes you propose. 
Your narrated presentation should be 5–6 minutes in length.
Your Change Implementation and Management Plan should include the following:
An executive summary of the issues that are currently affecting your organization/workplace (This can include the work you completed in your Workplace Environment Assessment previously submitted, if relevant.)
A description of the change being proposed
Justifications for the change, including why addressing it will have a positive impact on your organization/workplace
Details about the type and scope of the proposed change
Identification of the stakeholders impacted by the change
Identification of a change management team (by title/role)
A plan for communicating the change you propose
A description of risk mitigation plans you would recommend to address the risks anticipated by the change you propose.

An executive summary of the issues that are currently affecting your organization/workplace (This can include the work you completed in your Workplace Environment Assessment previously submitted, if relevant.)
Use general information to create a background slide (remember that the first paragraph of an APA paper is understood to be the introduction – this would also hold true for a presentation but remember to use key words rather than sentences). Be focused and talk about the problem that you will address; do not get sidetracked into presenting information that is not directly part of the problem. After a general description, you can add a second paragraph that describes the situation at your institution. Be factual and use data to support your opinions!
A description of the change being proposed
I should be able to clearly see the link between your executive summary and the change that you propose! For example, rather than talking about the nursing shortage, it may suit your purposes better to discuss retention (this is an example and you do not have to address this topic!).
Justifications for the change, including why addressing it will have a positive impact on your organization/workplace
What benefits do you anticipate happening if your change is implemented?
Details about the type and scope of the proposed change; Identification of the stakeholders impacted by the change
Be specific in this section. What steps will be involved in carrying out your change? Thing about this in terms of a process; how will you measure your success? Who needs to be involved? Think of things like staff, administration, finance, HR, . . . Who has the authority to approve your proposal?
Identification of a change management team (by title/role)
Based on your identification of stakeholders, who needs to be involved in developing and implementing your proposal? 
A plan for communicating the change you propose
You cannot over communicate!! Who needs to be kept up to date with what you are doing; How can you increase the chance of success through communication?
A description of risk mitigation plans you would recommend to address the risks anticipated by the change you propose
What could potentially go wrong and how can you avoid this?
